Please note I do require screening for all new Clients. I will need a phone number (no VoIP) and either one of the following:
- A photo ID/Passport
- At least one recent provider reference (Must have been seen within the past 6 months. Provider must have a website, social media presence, and an email address associated with their sites. I do not accept Provider phone numbers.)
